Chris Duggan, PT, CCM, Owner

About Chris...

I founded Care Connections based on a lifelong passion for serving people. I am a licensed Physical Therapist with over 15 years of physical therapy experience, much of which involved treating the elderly. I have worked in multiple medical arenas including hospitals, rehabilitation facilities, home health agencies, and nursing homes.

In addition, I am a Certified Case Manager and a certified member of the Aging Life Care Association.  Becoming an advanced level member of this professional organization requires years of experience, certification by one of four credentialing bodies, professional references, and a baccalaureate or higher degree in a field related to caring for seniors.  For additional information regarding the requirements for members of the Aging Life Care Association, please visit their website by clicking this button...

On a personal note, my family has faced the kinds of crises that often prompt a call to a professional Care Manager. So…when I offer professional assessments and recommendations, please know that they are blended with genuine compassion and understanding because I have walked this road.

About Debbie...

I’ve had the privilege of being a registered nurse for 20 years, primarily working in critical care. Critical care nurses must be proficient in a wide variety of high-level nursing skills. We are trained to evaluate intensive care patients, administer care, recognize complications, and coordinate with other members of the critical care team. These skills are performed within an environment of strong emotions and stressed family dynamics.

As a Care Manager, I’m able to use my knowledge and skills in a different but equally important arena. I serve as an educated advocate for my clients, offering them the benefits of my experience with genuine compassion.

I became interested in nursing as a teenager when my father suffered from several strokes eventually becoming bedridden. As I helped care for him at home and in hospitals, I realized the importance of a knowledgeable, caring nurse. My philosophy in patient care has always been to offer the highest standards of care in a compassionate and understanding manner.

About Lisa...

I'm a registered nurse with four years of experience in a rehabilitation hospital where I had the opportunity to provide compassionate care for patients with a wide range of diagnoses. This was a great learning experience which emphasized the importance of teamwork and an interdisciplinary approach to care. I have an Associate’s degree in Nursing from Cleveland State Community College and a Bachelor’s degree in Sociology from Appalachian State University. I look forward to applying my knowledge and experience in order to advocate for clients and to help them obtain exceptional care.
